The Co-ordinator will be jointly responsible for the effective co-ordination of the Placement Support Scheme.
The scheme is in partnership with the Integrated Care Board and takes referrals from Health and Social Care professionals, families and clients directly. The scheme will provide support and assistance for patients and families who are looking for a change of accommodation (including residential care) to a more suitable residence for their health and needs.
The purpose of the Placement Support Scheme is to help individuals/family/carers by providing a range of information and support, including transport (where appropriate) and signposting to other services to enable them to make informed choices about moving home, residential care and assisted living placement choices.
