Kay takes referrals mainly from Rotherham Hospital Stroke Unit for patients within 12 months of having a stroke to support their recovery and facilitating a connection with their community to engage and fulfil their goals.
This enhances the well-being through a person led care plan, empowering individuals to live an independent life with a sense of purpose.

Social support for 11–18-year-olds in education.
A children’s social prescribing link worker will work one to one with the service user to establish ‘what matters’ to them and set up a plan/goal orientated pathway of support.
Referrals may come from School nurses, Children’s Centres, SYEDA , With Me in Mind.
